Position Title: Homeshare Supervisor

Compensation: $67,787 - 74,068

NATURE OF POSITION:

Under the direction of the Director, Participant Services, the Homeshare Supervisor will oversee, implement and monitor activities related to the Homeshare programs to enable a meaningful life for persons supported and enhance community inclusion. 

While this position is primarily M-F days, applicants must be open to a flexible schedule with the ability to work occasional evenings and/or weekends.

KEY RESPONSIBILITIES

This position has unique responsibilities and is suitable for a proactive individual who can manage multiple, individual case files, who is highly organized and possesses strong communication, technology and computer skills. 

Responsibilities include but are not limited to:

  • Provide care, services and supports consistent with the agency’s Mission, Vision and individual plans.
  •  Participate in the development and implementation of all procedures and individual support plans (ISP) and support each person to achieve their goals.  
  • Adhere to all legislative requirements as outlined in Quality Assurance Measures O.Reg 299/10 which includes visiting provider’s homes and/or workplaces of persons supported, both scheduled and unscheduled. 
  • Actively seek, encourage and value input from people supported in all areas pertaining to their life and lifestyle; 
  • Facilitate and foster a positive relationship between the people we support, their families and/or significant others, and their Homeshare provider. 
  •  Assist parents and/or Homeshare families and parents by liaising with alternative providers and coordinating supports such as community support programs, respite, long term care, mental health or medical.  
  • Recruit: Review employment applications, and oversee the selection of Homeshare families, Respite families, and Passport workers; coordinate, schedule, and monitor respite for Home Share Providers in accordance with provider contracts. 
  • Optimize communications between the agency and family members and/or significant others to ensure relevant information is mutually shared, as appropriate; 
  •  Maintain all administrative requirements as outlined in legislation.  

  • Approve Passport funding, ensure all expenses and reimbursements are met following billing procedures 

  • Complete payroll for Homeshare Staff, Respite Providers, and Passport Staff as applicable.  

  • Represent the agency internally and externally, on all matters related to Homeshare funding, programming, and related initiatives, serving as the primary point of contact; 

  • Participate in special projects as assigned. 

  • Participate in agency on-call as required
  • Meet organizational standards pertaining to quantity and quality of work performed on an ongoing basis, performing all work-related tasks in accordance with Ministry and Agency policies, procedures, legislation, regulation and directives, and identify non-compliance for resolution. 

  • Participate and comply with all health and safety processes and initiatives to ensure the Agency remains a safe workplace and report any workplace accidents/incidents or hazards immediately. 

 

QUALIFICATIONS:

  • Education is in a relevant field of study, with a minimum of two-year Community College Diploma and/or a minimum of 5 years work experience in developmental services. 

  • Required to complete all agency training as outlined in all Policy and Procedures. 
  • Ability to communicate effectively orally and in writing.  

  • Computer skills with proficiency in MS Office, and relevant data collection platforms. 
  • Knowledge of principles, methods, practices, related to Quality Assurance Measures. 
  • Ability to perform in a pressure environment, with frequent interruptions and short deadlines.  
  • Ability to communicate effectively orally and in writing.  
  • Ability to maintain a professional appearance and positive company image. 
  • Possession of a valid Ontario class “G” driver’s licence.
